United Healthcare provides health plans for individuals/families that choose plans that don't cover pre-existing conditions. UHC also provides Group and Medicare Options. Group plans can choose disability, life, dental and vision coverage.

Health Insurance decisions require that we collect information in order to make the right recommendation.
For individuals/families we need the following information gender....zip code of residence... (not P.O Box)
Dates of birth for all persons wanting insurance. Tobacco status of all persons to be insured. List of all medications, dosage and frequency and form. EX medication name, capsule, 20mg, 1/day.
The medication list is needed in order to compare the medications in the insurance carrier's formulary (list of plan covered drugs) Unless instructed otherwise by client, we will not recommend a plan that does not cover all the client's current medications.
In addition, we verify all current physicians that care for our clients are in the plan we recommend. List of physician and preferred facilities (hospitals et al) are also needed before an assessment is started.
What deductible, and copays do you want? Generally, the higher the deductible the lower the monthly premium.

MEDICARE
Medicare assessments require all of the information above and the following additional information.
1. Medicare claim number
2. Effective dates of Medicare Part A and Part B
3. Preferred Pharmacies (Two)
LIFE INSURANCE
Life insurance choices include Term 5 year, 10 year, 15 year, 20 year, 25 year and 30 year. These are temporary policies and end at the end of the term you select.
Universal Life and Whole life policies are permanent policies and they end at the death of the insured.
What is a premium that would be acceptable?
Information required to provide a life policy illustration include: the insured's gender, date of birth, tobacco status, height and weight, health questionnaire may be required depending on carrier and plan type selected. Some plans require a paramed which includes urine specimen and blood draw, blood pressure check and life benefit amount might require financials and a physical exam. Especially for death benefits over a million dollars.
